Eligibility Quiz
Are you ready to learn your health coverage options?
With just a few simple steps, you will learn your options and be given all the resources you need to sign up for your best health coverage plan!
Take our simple 5-question Eligibility Quiz (Don't worry, this is not a quiz you have to study for, you already know the answers!) and receive a personalized list of both the public and private health coverage options for which you may qualify!
Your personalized list of health coverage options will include:
- Program Information - Overview of the program(s) for which you may be eligible and a further description of the program details.
- Cost - Approximate monthly cost of the health coverage program. Please remember all services provided by Foundation for Health Coverage Education are free. The monthly cost listed here would be the amount you would need to pay to the public or private health coverage program.
- Sign-Up Check List - A list of the documents you will need in order to apply for the health coverage program, for example, proof of income, birth certificate or proof of age. All programs are different, so it is important to review the sign-up checklist to make sure you have gathered all the documents that are needed.
- Links to Start Applications - An application link is available for every program. In some states, including California, we have created a Master Application that helps you complete more than one program application by filling out only one document.
- Special Resources - A list of your state's organizations that may help with paying for the services that are not free.

Please note: Due to the number of public programs being
created and/or changed, tools may not include all plans. The tools and
services on this website should be used as a starting point for general
information. Seek professional advice about specific plans and programs
from representatives of those organizations.
